What We Offer

We offer a free visit and consultation by a Nurse Case Manager to assess and better understand your needs, address your concerns, and customize individualized care plan to meet your needs.

We incorporate clients’ life style, medications and medical history, behaviour, home, and environment in the development of care plan.

We use nursing process to provide ongoing care; conduct regular meetings and quarterly review of client’s care plan, and then provide update as necessary for continuation of care needs.
